Nursing Home Case Manager OBRA (Healthcare)

LifeWays is looking for a Nursing Home Case Manager to join our team of compassionate, driven professionals. In this role, you'll provide trauma-informed, culturally sensitive mental health support to individuals in nursing facilities and carry a small case load of community-based consumers. You'll help people navigate systems, access services, and build a life of dignity, independence, and hope.
What You'll Do

Complete OBRA psychosocial assessments and ongoing mental health monitoring
Develop and implement Individual Plans of Service (IPOS) using person-centered planning
Coordinate care and connect individuals with financial, medical, and community supports
Support transitions--whether from hospital to home, nursing facility to community, or school to work
Provide crisis intervention and post-hospitalization planning
Collaborate with a variety of providers to ensure holistic, consistent care
Advocate fiercely for the people you serve

What We're Looking For

A degree in social work, nursing, psychology, or a related field
Experience with case management and/or mental health services (preferred)
Strong organizational and communication skills
A passion for helping others live their best lives
