Case Manager Mission Impact (Finance)
1. Conducts intake process for associates with disabilities and/or barriers, needing case management services.
2. As applicable, provides support to associates using a person-centered approach to ensure successful job retention and skill development. Coordinates supports and advocates for the associates needs.
3. Provides a full array of case management services; with an outcome of job stability.
4. Builds and maintains a network of local resources, appropriate to supporting the needs of associates. Develops relationships with various partner agencies to support associates needs.
5. Collaborates with internal resources such as the associate's supervisor, Human Resources and
Goodwill Academy, to identify relevant supports.
6. Communicates and maintains good rapport with referral agencies, individuals referred, and
associates throughout the organization. Serves as a liaison with these stakeholders regarding the progress of program candidates.
7. Completes and reviews intake data, service plans, case notes, and other documentation in a timely,
comprehensive, and accurate manner.
8. Maintains program files in accordance with CARF, Source America, DOL and other agency standards.
9. Develops, implements, and updates individual service/development plans.
10. Collaborates with community employers and partners throughout the business area to identify job leads and to assist associates with external placement when appropriate.
11. Supports process development, by identifying continuous improvement opportunities in service logic model, based on experiences.
12. As needed, provides reporting and metrics to appropriate stakeholders.
Supervisory Responsibilities
The associate in this position does not have any supervisory responsibilities.
Education/Experience/Certificates/Security Clearance
• High School Diploma or GED required. Bachelor's degree in human services or behavioral health fields preferred.
• 1-3 Years' experience in a related field preferred.
• Social work or other related credential preferred.
• Valid state driver's license, insurance and satisfactory driving record are required.
• Required to pass state and federal criminal background and sex offender checks. Depending upon contract specifications, may also be required to pass CPS and LEIE background checks and other background checks as necessary.
• Ability to operate PC, proficiency with Microsoft Office including Word, Excel, and Outlook.

This associate may have access to Confidential Information (CI) and is required to be familiar with the Goodwill Privacy policy 10.23 related to the handling of CI, and follow all related procedures required to protect the privacy and security of CI.
Physical Demands/Work Environment/Work Conditions
• The associate is required to lift 20 pounds maximum with frequent lifting and/or carrying objects weighing up to 10 pounds. Walking or standing to a significant degree involves sitting most of the time with a degree of pushing and pulling.
• The associate works in an environment with frequent exposure to extreme atmospheric conditions (temperature, noise, fumes, dust, etc.).
• The associate is required to travel throughout the Goodwill service area.• Associate may be required to transport associates and/or equipment.• The associate must be available to work evenings and/or weekends as business needs dictate.
